VACCINE CLINIC UPDATE.
The clinic will be holding the annual flu vaccination, for adults 65 and older. They will also provide the Moderna, Pfizer and Johnson & Johnson vaccines. The forms you have filled out are in a secure location. They will be held until our next date. If you have filled the consent form out please do not fill out another one. I will be taking them to the clinic on the 16th of October.
You can continue to pick up the form at the Clubhouse on the coffee table right inside the double doors. Place it in the provided envelope, seal it, write “COVID-VACCINE” on the outside and put it in the black box outside the building on the left as you exit. TIME: 11:00 AM UNTIL 2:00 PM.
PLEASE BRING YOU ID AND INSURANCE INFORMATION.
YOU WILL BE GREETED AND SIGNED IN BY VOLUNTEERS, THEY WILL DIRECT YOU TO THE LIVING ROOM. WHEN YOU ARE CALLED FOR TURN, YOU WILL PROCEED TO THE BANQUET ROOM.
AFTER THE VACCINE IS ADMINISTERED YOU WILL BE ASKED TO SIT IN CHAIRS DESIGNATED FOR 10-15 MINUTES
WE WILL BE SERVING COFFEE, TEA, WATER AND COOKIES.
